Be on your best behavior this month and be on time. Then write that letter of apology saying that you apologise for your past behavior and will be punctual in the future.

You should just talk to your boss. My former boss hated when people would write him long notes and emails etc... for things they could have told him in 3 seconds. Most of the time I was the one reading them. Just go in, ask for a minute and explain why you have been late and reassure your boss that your tardiness will not be an issue in the future. A letter really isnt necessary.

Actions speak louder than words. Your boss will notice the improvement if you just follow through on your vow to come to work on time and work harder. No need to write a letter....just shape up or you will ultimately be fired.

If you don't like the employment situation, go look for a better one. It's not likely that you will permanently change your boss' attitude.

this is not something you handle with a letter you handle it in person your level of stress will not come through in a letter, plus, it makes you look weak if you put it in a letter, meet with your boss but for the sake of keeping good records, after you meet with him and come to an agreement, write him an email summarizing the conversation, the agreements made, and the future plan, thank him for his time, and send it, that way you have written proof it happened
