This is a purely mathematical question based on statistical analysis. The fact that it is being applied to cancer statistics in this case is fairly irrellevant. You will get much more useful answers by asking in a mathematics/statistics forum.

They usually refer to with treatment, whatever that treatment is. Most of the time they basically just count the number of people who have been diagnosed with the disease who are alive x years after diagnosis (for cancer, usually 5 years). For most diseases that we would do this, people would be treated so the data reflects what happens if you are treated. That said, there are some diseases where a lot of the data is untreated. For instance, prostate cancer is often slow-growing and no treatment is needed, so quite a few people who survive 5 years will not have been treated. For diseases like that, it's often useful to look at disease-specific data, as a lot of patients may die from other things. Probably the best way to think of it as the percentage of people who survive x years with whatever would normally be done. Also, keep in mind that this does NOT mean "you will survive x years"; the statistics show what is normal, but all cases are different.

That info is gathered, grouped and averaged. Many people survive longer or shorter than the stats.

I'm gonna give you a heads up about stats - the wonderful thing about it, is that any sixth grader can do it - it's just got a bunch of formulas that make it *look* complicated. Seriously, it's much easier than algebra...so what I'm going to suggest is talk it out. Pretend you are trying to explain this to a freind who knows nothing about needing a formula.... How would you find out the likelihood that someone would a certain number of years? What would you need to know first? What would you need to known next? If you get stuck, try talking it through backwards... Then you write out your formula. When I would get stuck I would make an example with a bunch of single-digit data points. Helped me grasp a lot of things. Good Luck.

the only definitive cure for lung cancer is through surgery, however it is not done on everyone, as not everyone is a suitable candidate i would presume your friend is in the 60-70 age group, which is not a really the best age to have radical surgery (as it is a major surgery and may involve removing the whole affected lung) and taking additional chemotherapies or radiotherapies to increase the effectiveness of the surgery would make the experience much worse. and with the cancer already at an advanced stage, makes the doctor to tend to steer away from the surgical option. even so, sometimes with radiotherapy or chemotherapy the tumor may shrink and downstage the tumor that the surgeons might consider surgery as an option later along the course
